Parish Mission & Outreach Information Form

The Mar Thoma Church Diocese of North America is mapping the mission and outreach work our parishes do in their local communities. We want to understand what you already do, the needs you see around you, and the organizations you work with, so the Diocese can recognize and learn from this work and explore how parishes can support and connect with one another in serving their neighbors.

This is not an evaluation of any parish. Small or occasional efforts are just as valuable to report as large programs.

HOW TO COMPLETE
• One form per parish. It is easiest if the Achen and the outreach or mission lead complete it together, with input from Sevika Sangham, Yuvajana Sakhyam or Sunday School leaders where they run projects. About 20–30 minutes.
• Use the past 12 months (November 2025 – October 2026). Focus on service to people in or near your parish’s own community; support for mission fields in India or elsewhere has its own question.
• Estimates are welcome. Choose “Not tracked” rather than guessing.
• Please do not name the people your parish serves or include personal details about them.
• After submitting you will receive a link to edit your answers.
